Need help with multi section report

But before I can ask for help with the report, I need help to know how to post a request and also how to add an attachment when my profile says that I am not allowed to add an attachment.

You can post a sample of the report here using [ CODE] and [ /CODE] tags.

No attachments, unfortunately, but if you have a model file you want to email me I'll PM you my email address.
